Abgestimmte therapeutische Assessment-Verfahren: Entwicklungsstatus und Ergebnisse am Beispiel des Ergotherapeutischen Assessments

Zusammenfassung
Einleitung: Ausgangsbasis für ein interdisziplinäres Assessment-Netzwerk sind die abgestimmten therapeutischen Assessment-Verfahren für Ergotherapie, Logopädie und Physiotherapie. Der Bedarf wird vor dem Hintergrund der ICF, der Interdisziplinarität der Rehabilitation und der Forderung nach kontinuierlichem datengestützten Qualitätsmanagement aufgezeigt. Exemplarisch werden einige Ergebnisse zum Ergotherapeutischen Assessment referiert. Methodik: Expertengruppen der jeweiligen Profession entwickelten nach vorgegebenen Kriterien zu Struktur und Intention die Assessmentverfahren für Ergotherapie, Logopädie und Physiotherapie. Die Qualität der Verfahren wird in dreistufigen Untersuchungen (Probelauf, Pilotstudie, Validierungsstudie) geprüft. Zur Implementierung werden Anwenderschulungen durchgeführt.
Ergebnisse: Die bisherigen Untersuchungen zeigen für das Ergotherapeutische Assessment eine hohe Praktikabilität und Akzeptanz bei den Anwendern (Pilotstudie, n= 329) und zufriedenstellende psychometrische Eigenschaften (Validierungsstudie, n=833). Die Pilotstudie für das Physiotherapeutische Assessment diente der Fallzahlberechnung für die Validierungsstudie 2002/2003. Das Logopädische Assessment ist inkl. Handbuch fertiggestellt und wird in einer Pilotstudie ab Oktober 2003 untersucht. In der Anwendung des bereits geprüften Ergotherapeutischen Assessments sind bisher insgesamt etwa 300 Ergotherapeuten aus Deutschland, Österreich und der Schweiz geschult worden.
Diskussion: Der Nutzen der abgestimmten therapeutischen Assessment-Verfahren für die neurologische Rehabilitation wird diskutiert und weitere Entwicklungsoptionen werden aufgezeigt.

Consistent therapeutic Assessment-Instruments: State of development and results instancing the Occupational Therapy Assessment

Abstract
Introduction: The consistent therapeutic Assessment-Instruments for occupational, speech and physiotherapy are the groundwork for an interdisciplinary Assessment-Network. The need is described on the background of the ICF, the interdisciplinarity of rehabilitation and the demand for continuing data-based quality management. Exemplarily some results regarding the Occupational Therapy Assessment are reported.
Methods: Expert groups of the respective profession developed the assessment instruments for occupational, speech and physiotherapy according to given criteria for structure and intention. The quality of the instruments is assayed by a three-step investigation (pilot run, pilot study, validation study). Training courses for user are performed for the implementation.
Results: Previous investigations of the Occupational Therapy Assessment revealed a high feasibility and acceptance by users (pilot study, n=329) and satisfactory psychometric properties (validation study, n=833). The pilot study of the Physiotherapy Assessment provided data for the power analysis of the validation study 2002/03. The Speech Therapy Assessment is completed inclusive handbook and the pilot study has started in October 2003. Up to now about 300 occupational therapists from Germany, Austria and Switzerland took part in training courses for user of the Occupational Therapy Assessment.
Discussion: The benefit of the consistent therapeutic Assessment-Instruments for the neurological rehabilitation is discussed, also further options of development.
