Die Anwendung der ICF in der Neurorehabilitation anhand des ICF-Modellblattes und der ICF Core-Sets

Zusammenfassung
Rehabilitation ist das multi- und interdisziplinäre Management der funktionalen Gesundheit einer Person. Die Erfassung der funktionalen Gesundheit ist deshalb essentiell für eine effiziente Rehabilitation. Die neue Klassifikation der WHO zur funktionalen Gesundheit, die Internationale Klassifikation der Funktionsfähigkeit, Behinderung und Gesundheit (ICF), ist die erste mit einer globalen Perspektive entwickelte und von den Mitgliedstaaten der WHO als verbindlich anerkannte. Die ICF beinhaltet auch ein biopsychosoziales Modell zur funktionalen Gesundheit. Während das Modell bisher schon viel Beachtung gefunden hat, ergeben sich für die Klassifikation in der klinischen Anwendung Erschwernisse aufgrund der mangelnden Praktikabilität. Daher wird der Erfolg der ICF wird auch von der Entwicklung praktikabler Anwendungsformen abhängen.
Bei der künftigen Anwendung der ICF müssen daher zwei grundlegend unterschiedliche Anwendungsperspektiven unterschieden werden: 1. die Anwendung des biopsychosozialen Modells der ICF, beispielsweise in Form des ICF-Modellblatts für multiprofessionelle Teams, 2. die Anwendung der Klassifikation, beispielsweise in Form von ICF Core-Sets. Sowohl die Verwendung des ICF-Modellblattes am Beispiel eines Patienten nach Schlaganfall als auch die Entwicklung von ICF Core-Sets werden dargestellt. Die ICF Core-Sets oder Kurzlisten von Kategorien werden im Rahmen eines Kooperationsprojektes zwischen der ICF Research Branch und der WHO entwickelt, um eine effiziente Beschreibung der für eine Gesundheitsstörung typischen und relevanten Kategorien der ICF zu ermöglichen.
Die endgültige Fassung der ICF Core-Sets für chronische Krankheiten wird nach der weltweiten Testung und Validierung beschlossen werden.

Applying the ICF in neurorehabilitation using the ICF model sheet and the ICF Core-Sets

Abstract
The assessment of functioning and health is the core concept for rehabilitation medicine. The new WHO Classification – International Classification of Functioning, Disability and Health (ICF) – supplies the first global perspective to define functioning and health. Since the ICF takes into account the bio-psychosocial model, it has been accepted by the WHO member states as a mandatory framework for describing functional health from an integrative point of view. Whereas the bio-psychosocial model is widely accepted, the use of the classification in clinical practice is limited by a lack of practicability. Therefore the acceptance of the ICF will depend on the development of practical application forms of the ICF.
When using the ICF, two different general approaches have to be taken into account. First, applying the bio-psychosocial model, i.e. using the ICF model sheet in multidisciplinary teams. Second, applying the classification, i.e. using the ICF Core-Sets for chronic conditions. Both the use of the model sheet and the development of the ICF Core-Sets were presented. ICF Core-Sets were developed in a collaborative project of our ICF Research Branch with the WHO. ICF Core-Sets can be defined as a selection of ICF categories. The ICF Core-Sets will cover the prototypical spectrum of limitations in functioning and health encountered in a specific condition. The use of the ICF in clinical practice will be thereby facilitated.
The final version of the ICF Core-Sets for chronic conditions will be developed after the currently ongoing worldwide testing and validation studies.
