Das Team in der Neurorehabilitation

Zusammenfassung
Neurologische Rehabilitation ist die rehabilitative Therapie mit neurologischen Patienten in einem multiprofessionellen Team unter Leitung eines Arztes. Die Mitarbeiter dieses Teams arbeiten eng verzahnt und mit intensivem Austausch interdisziplinär zusammen. Dieses primäre Behandlungsteam besteht im Wesentlichen aus Neuropsychologen, Logopäden, Ergotherapeuten und Physiotherapeuten inkl. der physikalischen Therapie, unterstützt durch eine co-therapeutischer Pflege und begleitet von einem Sozialdienst.
Weitere Therapieformen können hinzukommen. Ein sekundäres Team stellt die ärztliche Kooperation des Neurologen mit Nachbargebieten wie Neurochirurgie, Geriatrie, Psychiatrie, Orthopädie und Neuropädiatrie dar. Ein tertiäres Team existiert in Form der organisatorischen Kooperation der ärztlichen und ökonomischen Klinikleitung mit Kostenträgern und politischen Entscheidern. Jede Art der Rehabilitation ist organisatorisch multiprofessionell ausgelegt und wird vom Wesen her interdisziplinär gestaltet indem die Methoden sämtlicher therapeutischer Disziplinen nach gemeinsamer Absprache durchgeführt werden um Synergien zu schaffen. Dies gilt in besonders hohem Maße für die Neurorehabilitation, da gerade hier, meist nach einer Erkrankung oder Verletzung des Gehirns als zentralem Regulationsorgan, in der Regel mehrere ganz unterschiedliche
Störungsbilder kombiniert auftreten, angefangen bei sensomotorischen Störungen wie Lähmung, Spastik, Dysphagie oder Koordinationseinbußen, über Beeinträchtigungen der kognitiven Leistungen und der Sprache, bis hin zu psychischen Alterationen wie Angstzuständen oder Depression. Aus diesem Grund ist es ganz besonders wichtig, dass sich die einzelnen professionellen Teilbereiche eng abstimmen um die rehabilitativen Ziele zu erreichen. »Multiprofessionell« bedeutet in diesem Zusammenhang, dass Vertreter ganz unterschiedlicher Berufsgruppen auf einem hohen Kompetenzlevel mit dem Patienten arbeiten,
und »interdisziplinär« heißt, dass die einzelnen an der rehabilitativen Arbeit Beteiligten mit intensiver gegenseitiger Absprache tätig sind. Die sogenannte transdisziplinäre Kooperation ist die höchste Stufe der Kooperationskompetenz und beinhaltet die regelmäßige Übernahme von therapeutischen
Tätigkeiten aus einer Nachbardisziplin nach Absprache. Sämtliche Formen der Kooperation werden unter
der Leitung eines in allen rehabilitativen Bereichen versierten Arztes durchgeführt. Dieser übernimmt aufgrund seiner Profession die Verantwortung für sämtliche Therapien, darunter auch die Verordnung von Medikation und gegebenenfalls Zuführung zu akutmedizinischen Interventionen wie z. B. einer Operation.
Die vorliegende Publikation handelt nur das primäre – therapeutische – Team ab. Es ist geplant, die Kooperationsformen des sekundären und tertiären Teams gesondert zu publizieren.

Abstract
Neurological rehabilitation is rehabilitative therapy that is concerned with neurological patients in a multi-professional team under the leadership of a physician. The members of this interdisciplinary team work together closely with an intense exchange of knowledge and competence. This primary treatment
team includes specialists in neuropsychology, speech therapy, occupational therapy and physiotherapy (including physical therapy), along with social workers and co-therapeutic nursing. Other forms of therapy may also be included. A secondary team is defined by the cooperation of the neurologist with other medical fields, e. g., neurosurgery, geriatrics, psychiatry, orthopedics and neuropediatrics. A tertiary team exists in the form of organizational cooperation between the medical und economic clinic management with insurance companies and political decision makers.
Every kind of rehabilitation is basically multi-professional from an organizational viewpoint, and it is performed interdisciplinarily applying the methods of all therapeutic disciplines to create synergies. This particularly applies to neurorehabilitation, as in this field – generally following a disease or an injury to the brain as the central regulation organ – several different disorders occur in combination: sensorimotor symptoms and signs like paralysis, spasticity, dysphagia and loss of coordination; cognitive or speech disorders; or finally psychological alterations like depression or anxiety. Therefore it is particularly
important that the professional team members match and coordinate their therapeutic procedures to reach common rehabilitative goals.
»Multi-professional« in this context means that the representatives of the different therapeutic fields work with the patient on an advanced competency level, and »interdisciplinary« means that the members of the team work together closely with thorough cross-consultation between their disciplines. Transdisciplinary cooperation represents the highest level of team competency and includes regularly adopting the tasks and therapeutic procedures from other disciplines. All kinds of cooperation are performed under the supervision of a physician who assumes full responsibility for the rehabilitative therapy, including the prescription of medication and introduction of acute interventions, e. g., surgical procedures.
This paper is only concerned with the primary – therapeutic – team. The secondary and tertiary forms of team cooperation are planned to be published separately.
