Lotsen für Schlaganfall-Betroffene: Vom Modellprojekt zur Regelversorgung

Zusammenfassung
Schlaganfall-Lotsen sind eine mittlerweile gut erprobte Intervention, um Defizite entlang der Versorgungskette patientenzentriert zu beheben. Sie ermöglichen auch eine bessere Interaktion zwischen den Beteiligten der Versorgung. Auch in anderen Indikationen haben sich die Lotsen bewährt. Nun gilt es für Patientenlotsen einen Pfad in die Regelversorgung zu ebnen; das Innovationsprojekt LEX LOTSEN OWL wird dazu Eckpunkte erarbeiten.

Guides for stroke patients: From pilot project to standard care

Abstract
Stroke pilots are a well-proven intervention to address deficits along the care chain in a patient-centered manner. They also enable better interaction between those involved in care. The pilots have also proven their worth in other indications. The task now is to pave the way for patient guides to become part of standard care; the LEX LOTSEN OWL innovation project will develop key points for this.
