Zusammenfassung
Im Projekt OptiNIV [3] erfolgt eine klinische Begleitung und ambulante Mitbetreuung neurologisch schwerst betroffener Patienten mit außerklinischem Intensivpflegebedarf (AIP) durch Fachnachsorgeteams (AFNT) der neurologischen Frührehabilitationseinrichtungen. Für diese Nachsorgetätigkeit wurde ein Leitlinien- und Evidenz-basierter klinischer Pfad (KP) entwickelt. Dieser bildet medizinisch sinnvolle und notwendige Inhalte wie die festgestellten klinischen Defizite, Behandlungsziele und -Empfehlungen ab, die individuell erfasst, abgestimmt, dokumentiert und kommuniziert werden. Dabei wird besonderes Augenmerk auf die multiprofessionelle interdisziplinäre Zusammenarbeit gelegt. Ziel war die Förderung und Unterstützung der transsektoralen Teamarbeit der Fachnachsorgeteams mit neurologischer Spezialisierung mit den ambulanten pflegerischen und therapeutischen Professionen sowie Ärzt*innen. Damit sollte auch ein »Continuum of Care« dieser Patientengruppe qualitativ definiert und in der Umsetzung erprobt werden. Mit den AFNT-Mitgliedern wurden Interviews zu den Praxiserfahrungen aus dieser Erprobungsphase geführt, deren Ergebnisse Einblicke in die derzeitige Versorgungssituation und die Möglichkeit transsektoraler Zusammenarbeit gewähren.

Cross-sector clinical pathway for people with severe neurological conditions – the experts’ perspective

Abstract
In the OptiNIV project [3], clinical monitoring and outpatient co-care of neurologically severely affected patients with outpatient intensive care needs (AIP) is provided by specialist aftercare teams (AFNT) from neurological early rehabilitation facilities. A guideline- and evidence-based clinical pathway (CP) was developed for this aftercare activity. This represents medically sensible and necessary content such as the identified clinical deficits, treatment goals and recommendations, which are individually recorded, coordinated, documented and communicated. Particular attention is paid to multi-professional interdisciplinary cooperation. The aim was to promote and support transsectoral teamwork between the specialist aftercare teams with neurological specialization and the outpatient nursing and therapeutic professions and treating physicians. This was also intended to qualitatively define a continuum of care for this patient group and test its implementation. Interviews were conducted with AFNT members regarding the practical experiences made during this trial phase, the results of which provide insights into the current care situation and the possibility of trans-sectoral cooperation.
