Der Stellenwert von Musiktherapie in der Neurorehabilitation in Deutschland

Zusammenfassung
Die personelle Mindestausstattung im Bereich Neurorehabilitation umfasst im Kernteam neben Ärzten und geschultem Pflegepersonal die Bereiche Neuropsychologie, Ergotherapie, Krankengymnastik, Sozialdienst und Logopädie. In den letzten Jahren haben weitere Bereiche wie z.B. Musiktherapie zunehmend an Bedeutung gewonnen. Anhand einer Befragung von 139 deutschen Neurorehabilitationskliniken soll der gegenwärtige Stellenwert von Musiktherapie in der Neurorehabilitation dargestellt werden. Musiktherapie wird in 32% der deutschen Neurorehabilitationskliniken mit Schwerpunkt in der Rehastufe B und C (Frührehabilitation) angewandt. Das meist genannte Indikationskriterium ist der Interaktionsaufbau beim bewusstseinsgestörten Patienten. Daneben wird Musiktherapie bei der Behandlung autonomer Fehlregulationen, bei Funktionsdefiziten im motorischen, sensorischen und kognitiven Bereich, sowie zur emotionalen Stabilisierung, Krankheitsbewältigung und Verbesserung der Lebensqualität angewandt. Es besteht ein hohes Interesse am Einsatz von Musiktherapie, obwohl in weiten Bereichen systematische Untersuchungen ausstehen. Zukünftige Forschungsprojekte sollten klare Indikationskriterien zum Einsatz von Musiktherapie in der Neurorehabilitation und Standards der Anwendung und Dokumentation musiktherapeutischer Behandlungen entwickeln.

The role of music therapy in german neurorehabilitation

Abstract
Health personnel in neurorehabilitation comprises apart from physicians and professional nursing staff also neuropsychologists, occupational therapists, physiotherapists, social workers and logopedics. In the past years other professional fields such as music therapy have gained increasing importance. After questioning 139 German neurorehabilitation hospitals we review about music therapy in German neurorehabilitation concerning current situation and practices.
Music therapy is practiced in 32% of German neurorehabilitation hospitals, mostly hospitals with rehab Level B, the so-called »early rehabilitation«. Patients most often treated with music therapy are those with vigilance disorder. The highest ranked indication criterion is »establishing interaction«. Besides this, music therapy is practiced in the treatment of autonomic dysfunctions, sensory, motor or cognitive function deficits, emotional stabilization, coping, and quality of life. There is a high interest in music therapy despite the lack of systematic studies in many fields of application.
Further research concerning music therapy in neurorehabilitation should develop clear indication criteria, standards of practice and documentation, and compare music therapy with other therapeutic strategies.
