Zusammenfassung
Vor dem Hintergrund unserer langjährigen klinischen Erfahrungen in der neurochirurgischen-neurotraumatologischen Frührehabilitation stellen wir unser Dreiphasenmodell zur frühen sprachlichen Aktivierung schwerst hirnverletzter Patienten vor. Das Ziel unserer Therapie ist es, optimale Voraussetzungen zu schaffen, die den Patienten zu ersten Interaktionen und Reaktionen anregen, um relevante sprachverarbeitende Prozesse zu aktivieren. Die erste Phase unserer Therapie umfasst die Kontaktanbahnung zwischen dem Patienten und dem Therapeuten, in der zweiten Phase wird der Schwerpunkt auf die Entwicklung formaler Dialogstrukturen gelegt. In der dritten Phase steht die Entwicklung komplexerer Interaktionen und erster Handlungsabfolgen im Vordergrund. Wir integrieren Elemente der auditiv verbalen, der taktilen und visuellen Stimulierung sowie die rhythmische und prosodische Gestaltung der Sprache, die immer an eine soziale Interaktion zwischen dem Patienten und dem Therapeuten gekoppelt werden. Unsere Art der Therapie der frühen sprachlichen Aktivierung bietet eine strukturierte Vorgehensweise für klinische Neuropsychologen und Sprachtherapeuten, die mit schwer hirnverletzten Patienten in der Frührehabilitation arbeiten.

A therapy model for the early verbal activation of severe brain injured patients

Abstract
From the background of our many years of clinical experiences in the neurosurgical neurotraumatological early rehabilitation we present our three phase model for the early verbal activation of severe brain injured patients. The aim of our therapy is to provide optimal requirements enabling the patients to show first responses and interaction in order to activate speech relevant processes. The first phase of our therapy includes the setting up of contact between the therapist and the patient, in the second phase the main emphasis is put on the development of formal dialogue structures. In the third phase the development of complex interpersonal interaction and first programmed forms of activity are in the centre of attention. We integrate elements of auditory verbal, tactile and visual stimulation, rhythm and prosodic organisation of speech which are always linked to social interaction between the therapist and the patient. Our kind of therapy provides a structured procedure for clinical neuropsychologists and speech therapists working with severe brain injured patients in the early rehabilitation.
