Klinische Behandlungspfade: Instrument der Qualitätssicherung in der neurologischen Rehabilitation

Zusammenfassung
Die bundesweite Umstrukturierung in der Finanzierung der Patientenbehandlung nach dem Prinzip einer pauschalisierten, fallorientierten Vergütung und die Abkehr von dem verweildauerorientierten Erlös wird tiefgreifende Veränderungen in der Organisation klinischer Prozesse zur Folge haben. Unter den derzeitigen Krankenhausbedingungen im angehenden »DRG-Zeitalter« lässt sich das Spannungsverhältnis aus Qualität, Zeit und Kosten zugunsten einer adäquaten Patientenbehandlung fast nur unter Zuhilfenahme klinischer Behandlungspfade beeinflussen. Der Behandlungspfad kann als eine berufsgruppenübergreifende, klinikeigene Strukturierung eines Behandlungsablaufs auf evidenzbasierter Grundlage, die Patientenerwartungen, Qualität und Wirtschaftlichkeit gleichermaßen berücksichtigt, definiert werden. Die klinischen Behandlungspfade sollen den intensivmedizinischen und rehabilitativen Behandlungsablauf steuern (Prozessqualität), sind gleichzeitig das behandlungsbegleitende Dokumentationsinstrument (Strukturqualität) und erlauben die Kommentierung von Normabweichungen mit der Zielsetzung der kontinuierlichen Evaluation und Verbesserung der Patientenversorgung (Ergebnisqualität).

Abstract
The restructuring in the financing of patient care in the German health care setting according to the principle of the case-oriented refined diagnosis related group system, in relationship with the gradual withdrawal from remuneration by treatment duration, will lead to extensive changes in the way that clinical processes are organised. Under the current conditions of the approaching »DRG era« in the hospital sector, the conflict between quality, time and costs in order to achieve sufficient medical performance improvement, is almost entirely influenced by recourse to clinical pathways. The clinical pathway can be defined as a structured, multidisciplinary guideline of best healthcare practice for a particular diagnosis or procedure, which supports the recommendations of evidence-based medicine, and takes into account patient expectations, quality and cost effectiveness in equal measure. Pathways are commonly developed for local use within an institution. The clinical pathways are designed to conduct and coordinate the daily medical and rehabilitative course of decision-making (process quality), to form the means for documenting the medical handling (structure quality) and to permit commentaries about pathway variances in order to facilitate continuous evaluation and advance in the care processes (result quality).
