Die Umsetzung der ICF im Spannungsfeld von Kontext und neurorehabilitativer Intervention

Zusammenfassung
Der Artikel befasst sich mit der Bedeutung des Kontextes für den Rehabilitationsprozess und die interdisziplinäre Zusammenarbeit im Rehabilitationsteam. Drei Schwerpunkte stehen dabei im Vordergrund: 1) Es werden die Umsetzung der ICF im interdisziplinären Rehabilitationsteam, insbesondere deren Einfluss auf die interdisziplinäre Kommunikation sowie auf die Organisation der Zusammenarbeitsmodelle und die Kontextausrichtung von Rehabilitationsmaßnahmen diskutiert, 2) Der Nutzen der ICF als Ordnungssystem für den Zielsetzungsprozess wird dargestellt und 3) Es wird über Erfahrungen mit der »Shared Care« berichtet. Darunter versteht man ein ambulantes Rehabilitationskonzept, das auf einem Zusammenarbeitsmodell zwischen dem spezialisierten Rehabilitationszentrum und den regional basierten Rehabilitationsverantwortlichen beruht. Blickwinkel der Betrachtungsweise bilden der Umweltkontext und die Partizipationsziele.

Implementation of the ICF between contextual factors and neurorehabilitative treatment

Abstract
This paper deals with the impact of contextual factors for the process of rehabilitation and interdisciplinary teamwork. Key aspects presented here are: 1) Implementation of the ICF in daily practice of an interdisciplinary rehabilitation team. Thereby, special emphasis is put on possible approaches to interdisciplinary communication and to organizational models of collaboration as well as on inclusion of contextual factors into treatment procedures. 2) The benefit of the ICF-framework is shown for goal setting in rehabilitation, 3) Positive experience with the »Shared Care Model« is reported. This describes a model of cooperation in patient management among a specialised centre of rehabilitation and rehabilitation specialists responsible for the outpatient treatment of patients in their residential areas.
