Das »Shared Care Modell« als effizientes und qualitativ hoch stehendes Versorgungsprinzip in der Rehabilitation – Beispiel der Neurorehabilitation des Kantonsspitals Luzern

Zusammenfassung
Das »Shared Care Modell« beschreibt ein ambulantes Rehabilitationskonzept, das auf einem Zusammenarbeitsmodell zwischen dem spezialisierten Rehabilitationszentrum und den regional basierten Rehabilitationsverantwortlichen beruht. Dabei koordiniert und begleitet das spezialisierte Zentrum die Wohnregion-gebundene ambulante Rehabilitation, berät das regionale Grundversorgungsteam und bietet eine Tagesrehabilitation als kostengünstige, intensive Form der Rehabilitation an. Aufgrund unserer bisherigen Erfahrungen bildet das »Shared Care Modell« ein effizientes und kostengünstiges Modell zur Rehabilitation von Menschen mit neurologischen Schädigungen. Es überbrückt die kritischen Schnittstellen zwischen stationärer und ambulanter Rehabilitation sowie zwischen medizinischer und beruflicher Rehabilitation und gewährleistet die Kontinuität des Rehabilitationsprozesses.

The »Shared Care-model« as an efficient and high-quality rehabilitation concept

Abstract
»Shared care« describes a model of cooperation in patient management among a specialised centre of rehabilitation and rehabilitation specialists responsible for the outpatient treatment of patients in their residential areas. Thereby the centre coordinates, accompanies and advises the local outpatient rehabilitation services. In addition it runs a day rehabilitation centre for patients in need of an intensive rehabilitation program on outpatient basis. Shared Care proves to be an efficient and cost saving model of rehabilitation for patients with neurological impairments. It improves the processes of patient management during the critical phases of transition between inpatient and outpatient rehabilitation as well as between medical and vocational rehabilitation.
