Vergütungsrelevanz von Mehrkosten durch Multiresistente Erreger in der Neurorehabilitation

Zusammenfassung
Die Hygienekommission der Deutschen Gesellschaft für Neurorehabilitation (DGNR) hat Ende 2017 repräsentativ die Mehrkosten durch Multiresistente Erreger (MRE) in der Neurorehabilitation erhoben. Diese belaufen sich auf 418 € pro Patient und Tag und setzen sich aus 144 € an direkten und 274 € an indirekten Kosten zusammen [16].
Hier wird die nachfolgende Diskussion darüber dargestellt, wie dieser Mehraufwand finanziert werden sollte, damit betroffene Patienten weiter adäquat rehabilitiert werden können. Dabei dominierte die Einschätzung, dass die ermittelten Mehrkosten in Form eines MRE-Zusatzentgeltes in Höhe der Gesamtkosten, mindestens jedoch in Höhe der direkten Kosten geltend gemacht werden sollten. Im Bereich der Neurorehabilitation nach § 111 SGB V (Rehabilitationskliniken) sollten im Streitfall Landesschiedsstellen unter Berufung auf die Gesamtkostenkalkulation der DGNR angerufen werden. Im Bereich der Neurorehabilitation nach § 108 SGB V (Krankenhausabteilungen) sollte diese Kalkulation als Orientierungswert dienen für die Tagessätze oder MRE-Zusatzentgelte in »Besonderen Einrichtungen« (nach Krankenhausfinanzierungsgesetz [KHG] § 17b) und für Anträge an das Institut für das Entgeltsystem im Krankenhaus (InEK) auf Umwandlung der Komplexbehandlungsprozedur bei Besiedelung oder Infektion mit MRE in ein entsprechendes Zusatzentgelt im Fallpauschalensystem.

Compensation relevance of additional costs by multidrug-resistant pathogens in neurorehabilitation

Abstract
At the end of 2017, the Hygiene Commission of the German Society for Neurorehabilitation (DGNR) evaluated the additional costs of multidrugresistant (MDR) pathogens in neurorehabilitation. The costs amount to 418 € per patient per day, consisting of 144 € in direct costs and 274 € in indirect costs [15].
The following discussion presents how the extra costs should be financed so that affected patients can continue to be adequately rehabilitated. The assessment was dominated by the fact that the calculated additional costs should be claimed in the form of an additional MDR pathogen supplement on top of the total costs to at least cover the direct costs. In the field of neurorehabilitation, according to § 111 of the German Social Code (SGB V), which pertains to rehabilitation clinics, in the event of a dispute, German state arbitration boards should be appealed referring to the total costs calculation of the DGNR. § 108 of the SGB V (hospital departments) this calculation should serve as a rough estimate for the daily rates or additional fees due to MDR pathogens in “special institutions” (according to the Hospital Financing Act (KHG)
§ 17b) and for applications to the Institute for the Hospital Remuneration System (InEK) for the conversion of the complex treatment procedure in the case of colonization or infection with multidrug-resistant (MDR) pathogens into a corresponding additional fee in the case flat-rate system.
