Einsatz pflegewissenschaftlicher Kompetenz im Case Management stationärer neurologischer Rehabilitation

Zusammenfassung
Zur Behandlungsoptimierung in der stationären neurologischen Rehabilitation wurde in 2009 ein Case-Management-Konzept entwickelt. Die Übernahme primär patientenferner Tätigkeiten aus Medizin und Pflege sowie die zentrale Koordination des Rehabilitationsablaufs erfolgen durch den Einsatz von Pflegewissenschaftlern in der Position als »Rehakoordinatoren«. Nach sechs Jahren Praxiserfahrung erweist sich die Kombination aus pflegefachlicher Berufsausbildung und -erfahrung mit einem pflegewissenschaftlichen Hochschulstudium in dieser vielseitigen Position als effektiv.
Obwohl die Rehakoordinatoren eng mit den Ärzten im Team zusammenarbeiten, geben die Rehabilitandenbefragungen (n=600) Hinweise auf eine Wahrnehmung des Rehakoordinators als eigenständige Position im Rehabilitationsteam. Auch waren die Bewertungen der Rehakoordinatoren zu denen der therapeutischen Professionen vergleichbar.
Mit Hilfe pflegewissenschaftlicher Kompetenz gelang die Generierung eines neuen, auch im Rehabilitationsteam anerkannten Tätigkeitsfelds.

Implementation of nursing scientific competence in the case management of inpatient neurological rehabilitation

Abstract
For optimizing the complex treatment of inpatient neurological rehabilitation a case management concept was developed in 2009. Therefore, physicians’ and nurses’ tasks in non-direct patient care and the central coordination of the rehabilitation process were delegated to nursing scientists, who held in the position of »rehabilitation coordinators«. Each rehabilitation coordinator underwent a combination of three years nursing training, work experience and studies in nursing science. After 6 years experience with the concept, this qualification proved to be effective in fulfilling this diversified position.
Despite close team work of physician and rehabilitation coordinator a survey of rehabilitees (n=600) indicates the coordinator to be noticed as a distinct position in the rehabilitation team. Also comparable assessments of the rehabilitation coordinators compared to those of therapeutic professions were seen. A novel and in the rehabilitation team well accepted operating position was successfully implemented by involving nursing scientific competence into inpatient neurological rehabilitation.
